Sliding Fees
RMHC has a sliding fee scale designed to support eligible individuals at or below 200% of the Federal Poverty Guidelines. Reduced rates may be available for outpatient services based on family size and gross income.
What is the Sliding Fee Scale Program?
The Sliding Fee Scale is a federal program that offers a discount on certain covered services. Eligibility is based on household size and annual household income.
Determining Eligibility for Sliding Fee Scale
Eligibility is determined by filling out an application and returning the required documentation. If approved for this program, eligibility will remain effective for one year from the date our offices receive the application. After one year, eligibility must be established by re-applying. All applications must include the following documentation in order for the application to be processed:
- Proof of income from the last 30 days OR prior year tax documents
- Completed application
If you need help obtaining this, our office can help with a MNSure application at no cost to you.
Services covered
Our Sliding Fee Scale covers the following services:
- Crisis services
- Treatment planning
- Outpatient mental health and substance use
- Targeted case management
- Outpatient primary care screening and monitoring

Services that are court ordered are not eligible for this program.
