Client confidentiality is a serious matter, and we here at RMHC are diligent about protecting your medical record. Your medical record contains personal health information important for you to know and could include diagnoses, treatments, medications and more.

If you are requesting medical records for yourself or your dependent, submit the request:

• During clinical visits / instruction
• In-person at your place of appointment
